Grants and Development Offices - Best Practices for Collaboration

Does anyone have a comprehensive list of tools/checklists/resources that outlines best practices for collaboration between their grants office and development/fundraising staff?
A few related questions:
• At your institution, how do you determine whether grants or development staff take the lead on a restricted donor proposal?
• When development takes the lead, does your institution make them adhere to any internal reporting or accountability standards/guidelines? How do you track and keep them in check?
• How is credit divided between teams?
