I interpret the IRB rules the same way you do, Nancy--student evaluations of teaching and similar user surveys are not required to go through IRB review of any kind even exempted review.
I appreciate the logic of your response and will borrow it next time someone asks me. The best I've been able to do is a slippery slope argument: if the IRB rules covered every case of asking others questions, faculty would have to file a protocol for every test, every quiz, every assignment they give. Clearly ridiculous; consequently we draw the line at user surveys.
