policies on faculty consulting

We are currently upgrading our policy on outside employment, particularly as it pertains to faculty consulting. I'd like to hear what other schools require in the way of approval for engaging in consulting activities. Do you require that faculty seek approval for every consulting job or just for every entity they consult with? Need this approval be reviewed annually? Who has the authority to approve or disapprove? Is this information available to your Office of Sponsored Programs or the equivalent? Any written policies and/or approval forms would be greatly appreciated.
