Also, when get there, go to NIH iPDF forms page.  We're in the process of
modifying the page, but until then be sure to SCROLL down and open/download
the iPDF forms.  The ones you have selected were created by and housed at
the NIH.  They are "standard" PDF, which means beautiful typewriter masters,
as you found out.
