When I was in a microbiology department, funded about $3 million per year
from NIH, we NEVER used Continuation forms. I now have some investigators
that use them, some that don't. As long and the name is the header and all
pages are consecutively numbered, NIH doesn't seem to care. (And, I have
have proposals rejected by NIH for failure to meet some formatting
requirement, so I take this as an implicit acceptance of the use of blank
pages.)...Bill

Am I living in another world or what? How many of you have investigators
who have NEVER used the continuation page form but use blank pages instead,
with only their name as the heading and a page number at the bottom? Is
this acceptable or merely tolerated? I looked through PHS398 and could not
find an exception explained.
