The facsimilies of the front form pages are all fine, but I have never worked with a group, as I do now, that doesn't use the continuation page. It's so easy to have it preformatted. I'm not going to insist on a change (probaby would make more work for support staff), but I was curious about others' experience. Thanks for the info.

Aside from margins, fonts, spacing, etc...
Make sure that facsimiles of the forms do not increase the size of the
abstract box on page 2. Big no no!
