I have a number of investigators who use blank pages, and I have never had
anyone from NIH comment on their use. Since several of the PIs have had
long-term NIH funding, I haven't insisted that they change. I do warn new
people that if they use blank pages, they should format the pages so that they
resemble the continuation page (margins, etc.) .

Am I living in another world or what? How many of you have investigators who
have NEVER used the continuation page form but use blank pages instead, with
only their name as the heading and a page number at the bottom? Is this
acceptable or merely tolerated? I looked through PHS398 and could not find an
exception explained.
