A few months ago someone asked list members for questionnaires that
assess faculty satisfaction with sponsored programs offices.  One list
member even sent me a copy of such a survey which I have, alas,
misplaced.  So--I'd like to get copies of  such questionnaires from
anybody who's used them, preferably as an attachment to e mail, but mail
and fax are also fine.  Thanks.
