I am the preaward manager in the Gifts, Grants & Contracts office (a
central administration office) of a medical school.   We're in the process
of updating our Proposal Transmittal form - the internal form which is
submitted to our office with each research proposal.   This form contains
general information about the proposal (PI name & dept., title, sponsoring
agency, type of proposal, etc.), assurance/certification information (human
subjects, animals, conflict of interest disclosure, environmental safety),
and is signed by the Principal Investigator and Department Chairperson.
We would like to take a look at proposal transmittal forms which are being
used at other institutions, and are particularly interested in how the
environmental safety approval information is approached/requested on this
form (radioactive substances, chemical carcinogens, infectious materials,
recombinant DNA).  We would appreciate receiving a copy of your current
proposal transmittal form from those of you who are willing to share it.
