The EPA Form 4700-4 is called the Preaward Compliance Review Report for All
Applicants Requesting Federal Financial Assistance. Although its content
doesn't seem to apply to research grants, I was told by an EPA Grants
Manager last week that it is required by their Affirmative Action/Equal
Opportunity Office. She said, "Just fill it out as best you can...."
