Based upon my past work experience with surveys, the questionnaire and reply
card should be submitted together.  A self-addressed envelope should be
provided (it need not be stamped).  If they are interested in participating
in the survey they will add the stamp.

The survey should be attractively designed, even put on colored paper.  There
are lots of artistic fonts you can use with word processors these days.
 These features catch the reader's eye and could bring in a 20% rate of
return...which is very good.
