I used OmniForm with great success.  Have created the NEH forms with it for the
 Seminary.  NEH
has the forms on their web site in pdf format, but all you can do with that is
 print and stick
it into a typewriter.  Obviously, NSF and NIH don't have too many opportunities
 for Talmud
scholars, so I haven't played with those forms.

But, I did a Medicare billing form for the nursing home my wife works in.  Her
 finance guy
erected a monument to me.  He got a copy of the program on my suggestion, and
 has successfully
used the form to submit and also has set up a protocol to transfer all the data
 to ACCESS on a
regular basis.  Has saved him tons of manual work.

I'm still using the Windows 3.1 version -- he's got 95 running and likes the
 added features.
