Has anyone OmniForm or OmniForm Filler for the development of internal
electronic forms?  If you have, I would like to know your impressions of
the program and what platform (e.g. Windows 95, Macintosh) you are
currently using the program with?  Have you been able to translate
OmniForm information into a free-standing database?

Has anyone developed any forms that investigators can fill out on the
Web, which also integrates with your department's database, and allows
for an investigator to print this form to his or her own personal laser
printer?
