Has anyone set-up faculty evaluations for the Community of Science
and/or SPIN on their web site?  I would be interested in hearing from you.
I would especially like to hear how you went about setting up the
evaluation (s), and what problems you encountered.  If anyone has
evaluation forms they might have developed, perhaps even a slick web
form they would like to share, I would appreciate if you could send me a
copy either by E-mail or by Fax.
