Thanks to everyone who faxed me a copy of the DD 1419 - DOD Industrial
Plant Equipment Requisition.  I appreciate your help.

While these were coming in we finally found it in the CCH DFARS on page
26,074.  One catch....it's the Dec, 1993 edition of the form.

I mention this only because ALL the copies that were faxed to us are Feb
1968 editions.  You will notice that the 12/93 edition states that
"Previous Edition May Be Used."  However, this (apparently) means the March
1989 edition, only, because the 3/89 edition states that all "Previous
editions are obsolete."

So, theoretically, for those of you with the 1968 edition.....you need to
get the '89 or '93 editions.  Now that we've found it, we'll be happy to
send it to anyone who needs it.
