Two people expressed interest in my posting about OmniForm.  I have created
the NEH forms with this software.  The file format is proprietary making it
impossible to convert the actual forms for use in word processors or
spreadsheets.

I'd like to suggest that if people on the list start using this program for
applications forms, we should share our files.  I'm more than willing to make
the NEH form available.  OCR is not perfect, and it takes some work to clean
up the scanned form.  No point in duplicating that effort if people on the
list have already done it.
