I am having this same problem.  I had actually killed, or so
          I thought, my original "subscription" when I left New
          England Deaconess Hospital 3 months ago.  I resubcribed when
          I got to Harvard 2 months ago.  I did have some trouble with
          the confirmation process.  I think it was at that point that
          the duplication occurred.
