At Western Illinois University, we require every potential
 principal investigator, regardless of funding source, to
 complete a checklist which consists of 4
 questions.  Unless one or more of the answers on the
 checklist is "YES," the applicant need not complete the
 full Statement of Economic Interests.

 When an award comes in, we check our files to ensure that the
 checklist/Statement of Economic Interest is on file and up to
 date.
