I tried to email you directly but was unsuccessful.

The packet (from STIS) is 29 pages.  I downloaded it in the .pdf format and
then had to load Adobe Acrobat to print out.  Do you have the narrative and
JUST need the forms (13 pages)?

Or, this is an option if you are unable to ftp and use MSWord 6.0:  I could
ftp that document to myself and then send it to you as an attachment.
