Although the original HHS 568 is a two-part carbon form, we have been
successful in sending them a completed and signed photocopy of the form.
Your grants management officer at NIH should be able to fax you one for
your completion. If you don't/can't get one from them, I can fax you
one. (I know what you mean about being asked to complete close-out on
long-completed grants. I have received about a half-dozen requests in
the last week or so. I assume they have been auditing files and are
finding some missing pieces!)
