HHS has just informed us that we have not submitted an HHS 568, 'Final
Invention Statement', for a long-expired NIH grant.  I can't find any
reference to such a form in any of my NIH/PHS/HHS stuff.  (We don't do
a lot of work with these agencies.)  Does anyone know where I can find
this form?  Is it available on WWW or a gopher somewhere?  Or can
someone fax me a copy (715/425-3185)?  I would appreciate it greatly.

(I suggested to our grants accountant that we send NIH a note to the
effect that our entire budget for that grant was our final invention.  She
was not amused.)
