Baylor College of Medicine (BCM) and the University of California, Los
Angeles (UCLA) have prepared several renditions of the Public Health Service
(PHS) Forms 398 for use with the following software packages:

-Microsoft Word 6.0a and Excel 5.0 for Windows
-Wordperfect 6.0 for Windows
-Microsoft Word 5.1 and Excel 4.0 for Macintosh

Although a Face Page is included in the MS Excel 5.0 for Windows version,
please note that it has not been approved by the NIH.  The other software
packages do not include Face Pages.

You may download these templates from the following web site:

If you prefer to receive diskettes instead of downloading the templates from
the web, we are prepared to send out a "reasonable" number of diskettes.  To
order one, be sure to specify whether you want the PC version or the
Macintosh version, and give your complete name and address where the
diskette should be mailed. (See request instructions at the bottom of this
message.)

We hope you will find these templates useful, however, neither BCM, the BCM
Office of Research,UCLA, the UCLA Sponsored Research Office, nor individuals
who created or revised the templates may be held responsible for any
specific or consequential problems associated with the use of these templates.
