We review and modify all consent forms to insure they meet the needs of
our institution.  It is a rare occassion that a sponsor's consent form
will meet the needs that our legal department requires.  The original
becomes a part of the patient record and is kept with the patient's
permanent file.  We talked about having the original sent to our office,
but found that copies did not always make it to the record with originals
would.
