At Armstrong State College in Savannah, GA, we require the PI signature on the
IRB application.
Our transmittal form requires the PI signature, and signatures from the
Department Head, Dean, Academic VP, and the VP for finance.  On the transmittal
form, the PI indicates, among other things, whether there are human subjects
involved and if so, attaches the IRB approval.  No proposal should ever (as
opposed to 'does ever') leave campus without approval.
