After re-reading my original response, I believe I left out quite a bit
of pertinent information. Our process includes a review process similar
to Cooper Hospital. Changes are made throughout the year as deemed
necessary. We too have a form outlining similar questions that PI's must
submit at their designated renewal time. All information is included in
the agenda for committee members to review.
