We interested in developing an internal review form/checklist for Material
Transfer Agreements that would parallel the purposes that  our Proposal
Transmittal Form serves for external proposals.

The kind of information we have thought about would include technical and
business contacts, addresses, approval of department chair/dean (including
a statement about risk assessment, laboratory safety, etc); whether the PI
has any existing agreements that involve the same or derivative materials;
any related commercial licenses, pending patents, etc.

Does any have such a checklist or similar internal review criteria for MTAs
you would be willing to share?  Thanks.
