Templates fot PHS398 and other forms are available by ftp from
ftp.u.washington.edu
they are in the pub\user-supported\templates directory.
The forms are available in Word/Windows and WPWin formats.
In addition, budget sheets are available in Excel, Lotus and QuattroPro
formats.
The files are available in self-extracting as well as uu-encoded form.
