Computer generated application forms

We too are trying to develop or obtain a program to generate the
PHS 398 and possibly the PHS 5161. We have found the "packaged"
products announced in SRA newsletters and the like to be a bit
pricey. Also, compatability is a big problem since we have many
investigators using different spreadsheets and word processing
programs. We HAVE found out that NIH will evaluate any final
form that we generate and approve it (for sometime we had been
under the impression that the 398 has to be submitted on their form
because they were undertaking a project to optically scan application
forms...) apparently, this is changed and NIH will review and approve
for use forms generated at the institution. I am in the process
of getting more details on this.
I would be interested in what others are doing in this area as well.
