As a small institution, we generally accept whatever a larger institution wants to use but we look for AOR commitment to:
- budget and scope of work for sub
- prime project sponsor, title and dates
- "enter into appropriate agreements" and comply with all sponsor policies
- standard debt, debarment etc assurance
- for NSF or PHS, the appropriate FCOI language
Increasingly our primes are using a standardized form (FDP maybe?) - we've now seen similar forms from Yale, UVM, Univ Alaska and maybe other primes. We will probably consider requiring this form in the future.

Can you please share what your sponsored research office requires in terms of institutional endorsement for subawards for NIH and other sponsors? We are considering substituting/accepting a NIH or NIH-like face page as a substitute for an institutional Letter of Endorsement and would like to know what other offices use/accept.
