NSF,DOE, etc.: Required statements on publications

Hello colleagues,
Regarding required acknowledgment/disclaimer statements by NSF, US Dept of Energy and other federal grant sponsors:
1) What types of grant related publications are subject to these requirements? Do those include student posters and web sites?
2) Do any of your institutions have an internal policy that addresses how to comply with these requirements?*
3) Does it matter if the grant related publications are paid for with grant or non- grant funds?
4) More specifically about grant related web sites: is one acknowledgment/disclaimer statement posted on the web site sufficient, or does the same statement also need to be included on each of the publications which are shown as links on the same web site?
