If the research is funded by PHS funds, then yes, under 42 CFR 93, the
institution receiving the funds is obligated to report, etc and required to
have institutional policies. It's one of the assurances that either must
be on file with ORI or must be included with the forms that get submitted
with applications for funding.
I don't know if the institution receiving PHS funds must include misconduct
related to research that isn't funded by federal money in the reports.

Is an entity that receives PHS funds required to report suspected research
misconduct?
At your university is this a requirement? And must you report also to ORI?
