Charlie,
My take has always been that it is to distinguish fellows/trainees from employees.
They are not working for a salary, they are in training and receive a stipend to keep the wolf from the door--consequently they are paid at the beginning of the month in anticipation of their expenses rather than at the end of the month in consideration of hours worked.
Also, in response to your original query, at my institution 99% of the fellowships are submitted in the name of the mentor and yes I have seen them submitted in expectation of the fellow's arrival/prior to affiliation with the institution.

Is it generally accepted that applications for post-doc fellowship
training may be made on behalf of PhDs who do NOT, at the time of
application, have any affiliation with the institution submitting the
application?
I.e. the commitment of the institution is based on its relationship with
the sponsor/mentor under whom the training will occur?
