I'm attaching policy/procedures and a request form that I drafted a number
of years ago for a PUI. We had a high indirect rate (on a salaries and
fringe base), so junior faculty who were pursing small capped grants who
needed the funds for summer salary lost more than half their "buying power"
if we insisted on applying the full rate. We put the policy in place to
help those individuals get started in research careers and also to have some
equitable way to evaluate waiver requests. Note that we didn't approve all
requests, especially those that came from experienced researchers on federal
funds whose only argument was the "competition" one

Our institutional practice has always been to recover full F&A when we can
and to accept what ever a specific sponsor allows (US DOE 85 mostly,
foundations mostly 0%, etc.) we also from time to time have allowed waivers
with strong justification. We are now attempting to place this into policy.
Do any of you have policies which state when an F&A waiver is allowable and,
if so, what the criteria are.
