Is this what you're looking for?  We use a Notification of Award for each individual award we receive.

Hello Everyone,

We are compiling a list of essential elements for an award-specific,
individualized summary document to be given to PIs/PDs and their dept.
administrators for their externally sponsored awards and contracts.  Do
any of your institutions provide such a document?

We checked the websites of several universities and found excellent
research administration resource pages.  The type of summary we're
looking for is a bit more specific than anything we've seen (or might be
likely to find) on an institution's sponsored programs website.

If you have a blank sample of elements you could share, we'd appreciate
it...
