Hi Jeanne -

For FDP forms (since we've now been dinged on it by several institutions) we issue them with the original # and add the Amendment #, i.e.: The NGA for Amendment #1 would be Attachment 4-1.

For our JHU prototypes, we label them with the Modification # and their current order, i.e. if the budget was Exhibit C in the original, but the only exhibit in Mod #1, we'll # it: Modification #1, Exhibit A.

That probably sounds way more complicated than it needs to be!

Hello!  We have had some lively discussions on numbering attachments on
subawards.  Using the templates provided by FDP, the question I have for
you is, if you amend a subaward and the amendment requires a change on one
or more of the original attachments but not all of them, do you number the
changed attachments sequentially OR do you number them as they were
originally numbered?  We have differing legal opinions!  How does your
institution do it?
