Testing/evaluation by a federal facility

Quick question...we want to send some materials to a federal facility
for them to do some testing/evaluation. When we contacted the
facility to start the process for a Confidentiality agreement, we
received a single page document entitled "Acknowledgement of
Responsibility Regarding Privately Owned Proprietary Information"
signed by the federal employee, acknowledging responsibilities under
Title 18, Section 1905 of the US Code. Basically, there is a single
paragraph where the employee acknowledges if they are convicted of a
violation of 18 U.S. Code 1905, they may be fined, imprisoned or
removed from office.
Have others seen this? And do you accept this instead of a more
traditional CDA?
