I've worked with two IRBs and neither required an annual recertification of
exemptions. Exempt means exactly that--it's exempt from review, now and in
the future. Of course, researchers are told that if they change the
protocol in a way that it no longer meets exemption criteria, then they will
have to submit the revised protocol for review. In most cases, I've found
that researchers will consult with the office for any little change they
plan, regardless of how minor. If the modification doesn't change the
exemption, we just respond with an email thanking them for the notification
and indicating that the protocol remains exempt.
