Greetings.
The "old" exemption #4 is really no longer germane. Check out the
following site for a very nice decision tree:

If the information is data or specimens that have already been
collected, are unidentifiable, publicly available, canot be linked to a
specific individual etc. than it is not human subjects research, even if
the subjects are still alive. If it is data on deceased individuals, the
decision tree says it is not human subjects research. So, you would
check "no" for human subjects.

I have errors relating to the Other project information because we did
not
include attachments to Women & Minorities; Targarted Planned
enrollment; and
children. This all seems due to not checking an exemption box under
human
subjects. We checked yes to human subjects and pending
application.then
there are other boxes i.e., #1 #2, #3 #4, etc.
The error refers to exemption number is not 4.the instructions indicate
a
website to go to for the common rule covering exemption #'s but the
websites
are down.
