Is the institution, one of the institutions, already registered with
the NIH Commons and with Grants.gov? If so then there is already one
SO and one AOR. Are you just starting the registration process to
get your institution registered with the Commons and G.g? If so then
make the decision asap as it might take a while for the "paper" work
to get done.
If you have submitted NIH applications in the past, which institution
did they come from? Who signed for the institution? The electronic
process should be the same as you have in place for paper. You can,
however, streamline it, but start with the paper process to get
registered.
Once you have all the official registrations in place, then have the
top SO and AOR create an SO and and AOR who is close to the proposal
approval process, someone in the grants management office. That
person should then do the submissions, and be named in section 18 of
the 424R&R form.
That latter process does not take much time so can wait until the
important registration of the entity is complete.

Our hospital is preparing to submit our first electronic proposal,
and we've run into a stumbling block in simply registering at
Grants.Gov and eRA commons. There is a discussion among several
people as to which hospital-associated entity should be responsible
for the application, who should be the signatory official, etc.
The 3-entities are (1) the Hospital itself, (2) the Hospital
Foundation, and (3) the Parent company, consisting of multiple
hospitals, real estate holdings, investments, etc.
In case it is important, I should mention the research is to be
performed in the hospital by hospital employees.
