Hi David,

As much as I hate to have yet another mandatory form, we too are using a
separate form for the grant applications and grant progress reports (see
attached document)and we're storing them in each individual file with
the relevant application and/or progress report.

Our current internal routing form doesn't have any room left on it to
add another thing so I could not incorporate these NIH certifications
onto an already existing form.

I'll forever be known at this institution as the person who developed
too many forms.

All of this paperwork reduction seems to be making more paperwork for
me!
