Greetings,

I am looking for examples of surveys or survey questions other
departments/campuses may have used with their faculty/staff/etc.  We are
the Dean's Office of the College of Agricultural and Life
Sciences--Research Division at UW-Madison.  We would like to solicit input
from groups with whom we interact, to determine their opinions of the
service we provide, as well as the service they desire, while emphasizing
state, federal, agency, etc. requirements.

Does anyone have examples of a quality improvement survey or suggested
questions focusing on Research Administration?  We want to focus on what
can we do to improve/assist.  Any suggestions/examples will be most
appreciated!
