Rather than extracting the pages, delete the pages you don't want and
save individual form pages as separate PDF files. Then you can "save as"
and insert them later wherever you want. I just tried it.

Yes, I already have Adobe Acrobat and know that I need it to work with
the
form. I am trying to extract the pages of completed information to
combine
with other Abobe pages into a grant proposal file for my own archive,
but
the locked Adobe file does not provide me with the option for
"extract."
