We have a faculty member who is interested in applying for an NIH Research Supplement to Promote Diversity in Health-Related Research (PA-05-015).  No one from our university has ever applied for this particular supplement, and the faculty member is looking for an example of a successful application.  Would anyone out there be willing to share an example, or do you know if NIH's website might have examples.  I looked, but was unsuccessful.
