One more suggestion I should have mentioned.  Before you write the survey, first determine what it is you want to know.  Think of it as a report.  What kind of information do you need in order to adequately assess the situation?  Only after you go through this exercise should you write the survey questions.  It will help you develop the appropriate questions.
