At my institution, the Public Health Institute, we have this happen every so often. We simple call the funder to advise them that the check needs to be made out to PHI, and return the check for a replacement, made out correctly. Word of caution: this caused somewhat of a dilemma since the PI included institutionally supported cost share in the proposal, yet the institution had no means of controlling or monitoring the funds received. Ideally, the check should have been signed over to the University.
