Definitely get the sponsor's approval up front. They may not approve
business class or the two seats under these circumstances. We have had
problems getting approval for business class before even though the
people involved had a documented legitimate medical reason. Some
agencies just aren't willing to pay for this sort of thing. We ended up
having to pay the difference from our funds.
