Dear Colleagues:

Our organization is starting to see more investigator initiated CTAs.
We are currently in process of developing a standard template for
investigator initiated clinical trials.  I don't want to reinvent the
wheel if there are good samples already available. Does anyone have a
standare/template agreement that they would be willing to share?
