New 398 modular forms and Other significant Contributors

Hello all,
Our PI is going to send samples to a lab in another institution to have
certain assays performed. We will pay about $14 per sample and were
intending to pay this as a PO. We have a letter from the head of that
lab giving us his estimate and indicating that his lab will do the
assays. Should the head of the lab be listed under other significant
contributors? Should we include his letter of 'collaboration" and/or
put something on the budget justification?
