federally funded clinical trials

I need help.
I am in need of some guidance on the financial aspects of federally funded clinical trials.
3 questions:
What price am I to use for patient care charges on federally sponsored clinical trials that are based on "cost"? a) our charge rate discounted to the cost to charge ratio on the medicare cost report? b) the medicare reimbursement rate for that particular test/service? c) something else??
What about when we receive a contract that has a cost reimbursable peice (for PI/study coordinator salary generally) plus a fixed portion per patient (or per test per patient) - what amount of cost is attributable to the grant and is therefore reportable on the Schedule of Federal Expenditures (A-133 audit)?
For the fixed price "reimbursement" per patient, what can we do with any residual? Can we do with it what we please? And I'm thinking I should set up another fund to "sweep" the residual into, so I'm not charging inappropriate costs to a federal award which is audited via the A-133?
