Jean,

Regarding your second question: I used to handle the administration of a
T32 award for a small division. Our post-doc's would obtain health
insurance on their own and make the payments. When they presented proof of
payment, I would have a check cut from the T32 account to reimburse them
for their expense.

There may be more than one mechanism for handling this; but if I remember
correctly, that is how we handled it.
